The following data sheets are useful in the special education classroom setting.

Data Rating Sheet

The data rating sheet can be used for individual objectives.  It is a quick and easy scoring system on a hierarchy of most to least assistance.  The rating definitions can be changed to best suit the given objective.  The data to be recorded can be at the end of a teaching session or at the end of a play time where objectives were integrated into the setting.  The rater can use a "rule of thumb" to provide a minimum of 3 opportunities or "trials" for the skill response then rate overall performance.

The Data Rating Sheet - Multiple Objectives is similar to the Data Rating sheet with the exception that multiple objectives can be placed on the same sheet.

The Skill Completion Sheet can be simply scored with a "+" or "-".  The objective and criterion should be very clear for scoring.

Trial Record Sheet

The trial record sheet is best suited to individual teaching sessions although it can be used in other settings as well.  The rater can score each trial by circling or placing a "+" on the numeral if achieved.  The rater can slash the numeral if the trial is not achieved.
